I hereby authorize the Clarksville Lake Country Chamber (CLCC) to list my business name to promote the CLCC and the Clarksville Lakeside Farmers Market (CLFM). I confirm that I have received a copy, read, understand and agree with the Rules & Regulations of the market. I understand that the rules may be subject to change before the market season begins each year and it is my responsibility to review the rules each year I apply as a vendor. I hereby agree to comply with all regulations governing the Clarksville Lakeside Farmers Market. I have attached or will provide the appropriate license/registration required and I hereby certify that the information included on this application is true and accurate. I further understand that any applicant who makes false statements or representations of certification in this application shall be subject to a fine and/or revocation of permit. I understand photos submitted will not be returned but may be used to promote the farmers market.
